Home Health
Sanford Home Care Canby is a Medicare certified hospital-based agency. Home Health provides convenient health care to patient’s right in their own homes. Services available include skilled nursing, rehabilitation, and home health aide services. These services are available to all ages and are provided through Medicare, Medicaid, County Waiver, insurance, and self-pay. Our service area encompasses a 30-mile radius of Canby including counties of Yellow Medicine, Lincoln, Lyon, and Lac qui Parle in Minnesota and Deuel County in South Dakota.
The Home Health Agency is located in the Hentges Center at 11 St. Olaf Avenue South.
Home Health Hours:
• Monday thru Friday............8 am to 4:30 pm
• Saturday and Sunday.......8 am to 12 noon
